Community First is committed to the wellness of the person served, Their families and other stakeholders through prevention, intervention, and education. 

Community First’s philosophy is always person-centered which means we inspire and support individuals to become productive and fully engaged and all aspects of their mental health care through, input, education and communication, while creating opportunities for wellness, independent living and working citizen in our community.

Skill-Building for Daily
Living Activities

 

such as Independent Living skills, Coping skills, Improved School Performance skills, Symptom Management skills, Accessing Entitlement skills, Social skills and Communication skills.

 Rehabilitative
Coordination

 

 such as referrals for ICD 10 clinical evaluation and therapy, psychiatric evaluations and counseling, psychotropic medication management, and accessing resources within the community.

Rehabilitation Counseling
for Adults

with gender-specific mentoring for children and adolescents.

On-site and Off-site
Activities

 

such as community excursions for social skill building, and summer activities (minimum 2 days a week for children 12 and under who are active clients) activities under professional supervision.

PRP: Psychiatric Rehabilitation Program is a program for adults, minors and adolescents with serious and persistent mental health illness. Trained  Psychiatrist Rehabilitation Practitioners provide services which include: 
Rehabilitation Assessment

 

evaluates the person served capacity to make important choices in their lives such as where to live, learn, work and where to socialize. If skills or resources are needed, the staff is qualified to teach or provide them.

Enrichment Support

 

 is provided to the person served to prevent re-hospitalization and crisis. On-site and off-site activities, group and one-on-one support are provided to achieve this goal.

Basic Rehabilitation

 

is provided when a person served is having difficulty adjusting to his/her current situation. Staff will perform rehabilitation in the environment of need as indicated in the assessment. 

Case Management

 

is a service that helps the person served to obtain entitlements and other community resources.

It is the goal of Community First Health Care Services, LLC (Community First” or “the company”) to provide quality mental health care services to those in need of such. Community first seeks to open and maintain its offices in urban communities, places where mental health care services are as needed as they are lacking in availability. Community First will provide both on-site and offsite program services, to maximize the delivery and understanding to the clients and the community of PRP services. Community First seeks to make these services available and consistent, in an effort to help adults and minors with DSM-5 related illnesses fight mental illness and the conditions that emanate from them, rather that be a crime, imprisonment, family separation, homelessness, unemployment, poor school performance, drug addiction and any other scourge 

drug addiction and any other scourge that can be tied to poor mental health, allowing our clients to return to their communities as full, hard-working productive members of society.  This type of outreach and recovery of necessary living skills envisioned by Community First serves to help the individual with internalizing skill building/life skills, it returns a citizen to productivity to themselves and possibly their family, returns a human being as a productive workforce member and reduces the homeless population by 1, a benefit to the City as a whole. It is the sort of extra-effort caring that sets Community First apart from other agencies and other visions, Community First seeks to put the health of the individual, as well as the health of the community foremost in its overall PRP efforts.
